Important information to include in your affidavit when visiting your nearest SAPS station:

  1. Full names.
  2. ID number.
  3. Matric or N3 exam number (if known).
  4. Year and month of matriculation or obtaining the N3 certificate.
  5. Name of the school or college where you matriculated or obtained your N3 certificate.
  6. Province where you matriculated or obtained your N3 certificate.
  7. Date of birth (if different from your ID details).
  8. Circumstances leading to the loss or damage of your matric or N3 certificate.

Note: You can only get an affidavit from a police station. You can NOT type it yourself.

A marriage certificate displaying maiden name - IF your current ID surname and surname on your Matric / N3 Certificate now differ due to marriage.
